If a person gives up sins during Ramadan but knows he will eventually end up falling into it after Ramadan due to his own weaknesses and addiction , is he a kaffir or a Munafiq and is his fast accepted ?

The fasting has nothing to do with future actions. And, a person who sins, will not be considered a munafiq or a kafir but is reffered as fasiq.
